New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.
Please note we ask for proof of address and photographic ID for the following reasons -
- To confirm you live in our catchment area
- To confirm we are registering the correct person
- To check the Health Authority have the correct information
- To prevent fraud (to stop people registering with many practices)
However if you do not have any ID we will accept your registration at Warmdene Surgery.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be found on the above 'Registering Online' tab.
Please allow one week for your registration to be completed. Medical treatment is available from the date of registration. Please contact reception for further information.
